You do not need to indicate your NRIC ... In the Address Portion.. It will be good if she fills in your address .. Then write your contact number after the address ... That will increase her credibility
Normally when u are back with her, you NRIC are recorded behind on her DE Card. That's happen to me all the time when I brought my wife. gf then into Singapore.
Whatever she can provide will increase her credibility into entering Sg BUT no necessary you sure can comes in ...

My suggestion is Don't ask this type of Qn and Get Answer here, BETTER call ICA and clarify this .. NO Joke if wrongly fill up and it would result in Refused Entry and/or a Ban. Don't KPKB here is u follow and result is otherwise ...
